Please fill out one form for every coach on the team. No matter if this form is good or bad it will help in the final scores for your coach to return the next year.

League Officers

Parents Requirements:

• Must Attend the General Membership Meeting

  • Livonia Civic Library Main Auditorium

 

• Fill out the Parents “Code of Ethic Form”.
• Sell tickets for the Casino Fundraiser
• All parents are required to sign a Code of Ethics
• There are no guaranteed playing times for any player by a head coach. The coaches will see the skills of the player and apply them to the positions that will benefit the team best. 
• Players Must play at their age group AGE unless approved by the league officers.
• Fill out the Coaches Review form at the end of season (on-line) and email to the league officers.  
• This form is used to evaluate your coaches from year to year.
• Livonia Storm Tournament:  Which will be scheduled on the June 20th weekend.
• It is Mandatory that parents work a minimum of two hours to support this tournament.
